Back in the day, the quality of your death was determined by how cleanly the executioner felt like slicing your head off and how sharp he had decided to make his axe. If you were unlucky, it would take a few extra blows, and it's precisely this kind of medieval barbarism that makes for a great Halloween costume.
Considering his line of work, one can't really blame an executioner for wanting to hide his face. Because of this, you're going to need a black hood that covers most if not all of your mug.
One popular version is essentially a black cloth bag draped over your head with two holes for eyes. This should be pretty easy to make out of any black fabric that you have laying around. Just make sure that no one can see through it.
Going with the theme of nondescript clothes, you're going to need a cloak that appears pretty generic. To be more recognizable, go with a stereotypical medieval cloak. You can find these easily online or at party stores.
Lastly, you're going to need the heavy axe that the executioner will use to lop off people's heads. Naturally, it's not advisable to get a real one, but there may be fairly convincing foam and plastic replicas that will pull your look together.
